Website inquiries and clinical records

This public website is an information and inquiry channel. It does not provide a patient portal or medical-record upload. Please do not enter patient names, symptoms, diagnostic reports or other medical records in the website inquiry forms.

Requesting access or a correction

For information sent directly to HeartLINK+, email info@heartlink.plus with the subject “Data protection request”. Describe what you are requesting and the correspondence concerned. Start with the minimum information necessary; do not attach identity documents or clinical records unless the team arranges an appropriate way to verify and handle your request.

Requests about patient care

If your request concerns care at a particular facility, contact that facility about its records and privacy process. HeartLINK+ can help identify the appropriate contact where it is involved in the service. For patient care, refer to the privacy information and consent process provided by the responsible clinical provider.

Your rights in Ghana

Ghana’s Data Protection Commission explains rights under the Data Protection Act, 2012 (Act 843), including access to personal information and requesting rectification. Rights and applicable conditions depend on the circumstances. Read the Commission’s guidance for individuals or contact the Commission if you wish to raise a concern.

Responsible communication

Please report a suspected privacy issue through the contact below, describing the concern without forwarding sensitive records. For implementation partners, data-handling responsibilities and appropriate transfer channels should be agreed before exchanging clinical information.

Information within a clinical pathway

Clinical assessment can involve identity, consent, symptoms, history, vital signs and cardiac findings. Diagnostics can add ECG traces, ultrasound images, reports and recommendations. Follow-up can record referrals, attendance, clinical actions and completion. The participating provider should explain which information applies to your care and why it is needed.

Clinical and research information

This website does not collect clinical records for research or model development. Questions about a particular care or research activity should be directed to the responsible provider or study team through the relevant privacy and consent process.
