Who we serve

One connected pathway. Different ways to begin.

HeartLINK+ works with the organizations that deliver and finance care, the experts who strengthen it and the researchers who evaluate it.

Facilities and health systems

The patient is already there. The complete pathway may not be.

Add structured assessment, essential diagnostics and specialist interpretation to the place where patients already arrive. HeartLINK+ contributes the workflow, H-TRIAGE™, reporting and navigation; the facility brings clinical space, staff and local physician responsibility.

Starting point and success measures

Starting point: Access, extended to Diagnostics as appropriate.

Success measure: Completion of the clinically appropriate next step, including diagnostics when indicated.

Deploy HeartLINK+
Employers and payers

Move beyond one-time health checks.

Connect assessment to diagnostics, specialist interpretation and a defined route into care. An employee or member who is flagged needs a reachable next step; sponsors need to understand completion as well as attendance.

Starting point and success measures

Starting point: An ACCESS program for a defined population.

Success measure: Completion of recommended evaluation, reported at population level only.

Deploy HeartLINK+
Governments, NGOs and funders

Strengthen access across communities and health systems.

Connect ACCESS sites, diagnostic hubs, experts and referral facilities. Local staff are trained, local clinicians remain responsible and performance is measured across the pathway.

Starting point and success measures

Starting point: A district or regional network beginning with one validated site.

Success measure: Referral completion and geographic equity of access.

Deploy HeartLINK+
Research and evaluation partners

Generate evidence from real-world care delivery.

Evaluate assessment, diagnostic access, workforce models and referral completion prospectively. HeartLINK+ Ghana delivers, G-ACT supports research and equitable access, and academic partners evaluate independently.

Starting point and success measures

Starting point: A defined evaluation question at an implementation site.

Success measure: Independent interpretation and peer-reviewed output.
